Health Care Homes Learning Days is an annual, in-person conference hosted by the Minnesota Department of Health. The conference brings together primary care teams, clinic leaders, community partners, and system stakeholders to share practical strategies that strengthen care coordination, advance health equity, and support integrated, patient-centered care across Minnesota.

Sponsorship Opportunities

Sponsorship opportunities provide targeted visibility and meaningful engagement while supporting access, equity, and the long-term sustainability of Health Care Homes Learning Days.
