Health Care Homes Learning Days is a high-energy, collaborative conference designed for clinics, care coordinators, public health professionals, and community partners working to improve care and advance health equity. Each year, more than 200 attendees come together to share best practices, learn from real-world experiences, and build meaningful connections across Health Care Homes and community systems. Join us at the Heritage Center of Brooklyn Center for a day of practical learning, collaboration, and inspiration you can bring back to your work.
Objectives:
- Describe strategies which improve patient and family engagement to support health literacy, manage chronic diseases, reduce risk factors, and address overall health and wellness.
- Identify innovative approaches and effective tools that enhance care coordination workflows and improve collaboration across care teams, payers, and community partners.
- Explore strategies that support long-term sustainability of the HCH care delivery model.
- Discuss key concepts related to implementing integrated care teams that support behavioral, physical, and social needs.
- Examine strategies that use information and community partnerships to advance health equity and improve population health.
Contact Hours Credit
This activity has been awarded 6.0 contact hours.
It is the responsibility of each participant to determine whether these contact hours meet the requirements of their individual licensing board, certification body, or employer, and to submit documentation for acceptance as needed.
Conference cost
Cost: $175 plus a 2.15% processing fee.

Cancellation Policy
- Full Refund: If MDH finds it necessary to cancel the pre-conference workshop or main conference, a full refund will be issued.
- Partial Refund (fee minus $25): If a participant needs to cancel, MDH must receive notification at least 3 business days prior to the first day of the pre-conference workshop or main conference.
