MEDIA RELEASE AND WAIVER OF LIABILITY

FOR PARTICIPATION IN LA84 FOUNDATION or THE PLAY EQUITY FUND PROGRAMMING

By agreeing to the above waiver I agree to voluntarily participate in an LA84 Foundation or The Play Equity Fund program or event. In connection with my participation in the Program or Event, I hereby grant to LA84 Foundation and The Play Equity Fund the right to take motion and still pictures of me and record my voice and any sounds made by me, and to obtain other information about me, including but not limited to my name, likeness, photograph, voice, dialogue, sounds, biographical information, social media (including, without limitation, texts, tweets, posts, photos, and any other form of platform and/or application I use to create or share content with others), personal characteristics and other personal identification (collectively, the “Footage and Materials”), and to use the Footage and Materials in connection with the Program or Event and any other publicity, promotion, information, awareness for the LA84 Foundation or The Play Equity Fund. The Footage and Materials and the program may be exploited throughout the universe at any time, in perpetuity, in any and all media, now known and hereafter devised, without any monetary compensation to me whatsoever. The rights granted herein shall also include the unrestricted right to edit the content and text of the Footage and Materials.

In consideration of such participation, I , the Participant, hereby release, discharge and indemnify the LA84 Foundation and The Play Equity Fund, its directors, officers, all program sponsors, consultants, athletes, coaching, other participants, volunteers and the successors, subsidiaries, affiliates, licensees, agents, assigns, officers, employees and trustees of the aforementioned entities from all liability for injury to the Participant or damage to property of Participant.
